Public Advance Care Planning Information Session!

National Hospice Palliative Care Week is May 3rd-9th. In recognition of this week, please join the North West LHIN Regional Palliative Care Program and Hospice Northwest for a discussion panel on Advance Care Planning Thursday May 7th from 7pm-9pm in the Georgian Room, St. Joseph’s Heritage 63 Carrie Street. Advance care planning is an opportunity for you to reflect on your values, and wishes, and to let others know your future health and personal care preferences so that they can speak for you, if you can't speak for yourself. An Advance Care Plan allows you to have a voice for a time in your life when you may not be able to speak for yourself, and the power to choose who will be your voice.

On May 7th panelist will discuss their role with Advance Care Planning and engage in discussion about how to begin this sometimes difficult conversation and keep it going. This event is free and open to the public . For more information please see event poster (click here) and share widely.
