FYI -- HPCO - Health Care Consent Advance Care Planning Community of Practice: Broad Membership Recruitment

HPCO officially launched the provincial Health Care Consent Advance Care Planning Community of Practice (HCC ACP CoP) last summer in response to the need for provincial resources on HCC ACP that utilizes an Ontario legal framework.  
 
The resource was created to support clinicians, administrators, caregivers, policymakers, researchers, educators and leaders who are committed to the promotion of HCC ACP in Ontario. 
 
To date, the CoP consists of a strong Leadership Advisory Team comprised of a diverse group of experts in the legal, policy, clinical, operational, knowledge translation and implementation domains of HCC ACP, which also includes representation from the Ontario Palliative Care Network (OPCN). 
 
The CoP also includes a designated Regional Champions Group comprised of one to two experts from each LHIN who have a lead role for promoting and implementing HCC ACP within their geographical area.  
 
Much time has been spent over these last few months preparing a strong CoP that can support requests for information, materials and resources; and building capacity for knowledge transfer, coordination and synergy.  This includes the recent launch of the new Speak Up Ontario Web site modeled after the National Speak Up web site to promote education, tools and practices which are compliant with the Ontario legal framework.
    
The CoP is fast becoming a Centre of Excellence for ACP in Ontario and are helping to support the integration of ACP into practice utilizing the Ontario Legal Framework.
