Learning Objectives:

· Describe regional and provincial palliative care initiatives such as Early Identification, implementation of the OPCN Health Services Delivery Framework, establishment of hospice-like spaces, and development of a local Integrated Palliative Care Clinical Program

· Highlight key tools and resources and how to access them

· Discuss how individuals and organizations can leverage this work to implement improvements in palliative care at the frontline level, including examples from around the region
